Boulder Wellness Network (boulder-wellness.net) is a collaborative effort to provide information and resources to the community of Boulder. All content is subject to review and corrections are appreciated. This is not in anyway a replacement for medical knowledge or expertise.
Boulder Wellness Network is not a health care provider in that it does not directly provide any medical services.
Currently, Boulder Wellness Network is a personally funded endeavor with limited services. We would like to develop the program over a multi year plan (timeline below) that would allow opportunities for growth, expansion of services to sustain a health and safe community.

Phase 1
Assessment & Planning

Develop & complete a needs assessment provided by a 3rd party organization

Establish strategy based on data from Needs Assessment, community input and Public Health information.

Identify resources available from town, county and state, federal rural health support

Work with the University Of Utah and Rural Health Program

Explore network opportunities with local providers

Build community health network based on a Village to Village model.

Offer information referral resource online through a collaborative community discussions.

Invite practitioners and mobile health services.

Secure physical location for administrative office and health supply distribution.
Phase 2
Development

Community training, scheduled health provider visits, workshops.

Develop care network with regional providers.

Assist our community with life planning, financial, home retrofits and medical information, discussions and referrals as needed.

Develop a First Responder CPR + Wilderness First Aid Training program for the community.
Phase 3
Implementation - Operating

Offer in-town health care options and services to community through partner providers.

Create rotating resident heath professional positions to support mobile practitioners and partnering clinics.

Provide trained local staff, assist health and safety needs and resources to provide in aging in place options and general assistance to our 65+ community.
